Please download the above PDF Membership Form, Release of Liability and Questionnaire, fill them out and return to 203 Harts Bridge Rd, Jackson, TN 38301 with your payment. You can also submit the Membership Form by direct email if paying with a credit card or the PayPal Donate Button. All membership fees will go to build the Temple, e.g. obtaining a building, festivals, community functions, etc.
